By completing the form below -

• I understand that Reiki is a gentle, energy healing technique that is used for alleviating stress, pain management, and to help remove tension.

• I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a veterinarian.

• It is recommended that I see a veterinarian professional for any physical or psychological ailment.

• I understand that Reiki can complement any medical or psychological care my animal may be receiving.

• I acknowledge that long term imbalances in the physical body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.

• I understand that Reiki is being provided by Liz Thomas at my request. I agree to hold Liz Thomas harmless and understand that she is not responsible for the outcome of the session.

• I consent for Liz Thomas to hold records of treatments.

In the UK, animal Reiki is legal as a complementary, non-invasive therapy but must not replace veterinary treatment. It is not considered veterinary surgery under the Veterinary Surgeons Act 1966, but practitioners must not diagnose, prescribe, or claim to cure. It is essential to have vet consent or assurance that an animal has been seen by a vet.

I have been advised by Liz Thomas (Reiki Practitioner) that I should consult a veterinary practitioner as appropriate.

Cancellation Policy:

Please note that once you have booked an appointment, it means that Liz has reserved time in her schedule exclusively for you. If you cancel your appointment less than 24 hours before it is scheduled to take place, you will be charged the full amount of the session or one session will be deducted from your pre-paid sessions. To avoid a cancellation fee, please provide cancellation notice at least 24 hours prior to your appointment via email at liz@reikiremedysanctuary.co.uk

I hereby certify that I have read this document and I understand its content. I am aware that this is a release of liability as well as a contract and I sign it of my own free will.

By signing this I give myself permission to advocate for myself to feel safe and comfortable while working with Liz Thomas.
